shift cable adjustment help?

shift cable to drive when checking the 6" from barrel to stud hole I cannot get 6" as the barrel is tight to the jam nut?
I can get there if I unscrew the threaded rod cable run through a few turns. what's the problem?

I have noticed that the shift cable linkage was loose. I tightened it back up but now find that probably 1 out of 2 times when I try to go reverse, the motor dies? can somebody help me for this i am very worried now.
