"My volvo penta 2003 has been doing strange things this past month. The problems started with bad fuel, which implied cleaning the tank. A few days later the engine would not start, then diesel filters were changed and cleaned, and the fuel system vented. The engine still did not work properly (awful sound and lots of white smoke) till suddenly it accelerated on its own to max rpm and remain like that for a few seconds. After that it started sounding perfectly. However, during the following days, the engine would again accelerate on its own to max rpm. It did so about 6 times, but it gave the impression the the problem was getting fixed on its own (somebody suggested air bubbles before the fuel pump...). In any case, besides those strange accelerations, the engine seemed to be running fine. until suddenly I heard the oil alarm and it stop. It has suddenly leaked *all* (ie 4+ lts) its oil (wetting mostly the rear part of the carter). The most strange thing comes now. I refilled the oil, started the engine (with some difficulty but not much), and now it seems to be running normally.
Anybody can explain what happened to my engine? How can I get sure that the oil will not come out again?"

I got the feeling that what might have happened is water/poluted diesel in the pumps that has seized the regulator/injection pump from working properly. Might even be lack of fuel. In later case the pump sleeve and element gets heated up and gets stuck at full opening. The engine oil might have been thrown out if the engine has been over reving.
A 'worse' senario is if Your oil pressure regulator has failed giving too high oil-pressure. New oil might have loosened it.
I would have attached a manual pressure reader and checked engine oil pressure to eliminate the later.

"Morten, thank you for the advice, but the acceleration episodes occured several times, even after a full change of fuel and filters. I had the
engine running at 1700rpm and sounding ok, and after
some 5' will accelerate on its own. It repeated that
several times and eventually seemed to have gotten ok by itself. However, following that it threw out all the 4lts of oil. I put the oil back and now seems to work fine. But just got checked the compression and the oil in the carter gets pressure... I know one possibility are the rings, but is it conceivable that a malfuntion of the oil pump may be putting pressure into the oil in the carter? Thank you!"

"It is possible that the oil pressure valve gives too high oil pressure, which should be checked out by using a manual pressure tester."

"This may or may not be of hel

"This may or may not be of help; I have had two cars that experienced the same problem (white smoke, lots of noise and self accelerating) in both cases it was the turbo causing the problem. Apparently the bearings had worn out and the lubricating oil was being drawn into the engine and burnt as fuel. If this is the cause I would suggest you get it fixed QUICK! I didn’t and all the bearings in my engine suffered as a result due to being starved of lubricating oil."
