Thanks to all for the probable culprits of my blown (melted hose)!
1. After checking my impeller, I found one of the "blades" was torn off and others looked to be melted a little on the ends AND the actual housing was a little melted through in one spot.
2. Pulled off the riser and hose to find the brand new flapper I installed with the hose was melted and hanging on by one ear hole.
3. Took off t-stat housing to find some rust and an ugly t-stat.
