6hp Johnson Flywheel came loose?

What did I do wrong here? I've just finished rebuilding the motor, ran it for a few tests over the last couple weeks adjusting the carb and now yesterday when free revving all I hear is " clank clank clank clank" and the darn flywheel popped up. It did damage the crankshaft groove a tiny bit but I might be able to get away with it. Any tips? Suggestions? I just torqued it down to 40ish lbs.
 
It was OEM

I just took the magneto plate off tonight and found that the cam bushing thing is cracked as well so I'll need that, a new flywheel and new key. Damn.....
 
Yeah i'll do that after I get the new parts, I think the dealership will want 35 bucks at least for that darn bushing key. I torqued it down to 40-45lbs on a torque wrench. We'll see what happens next time...

I do have 3 different types of plates (the yellowy gold brass) that came with different parts I ordered used from ebay , they all seem to have different hole positions? What are they used for aside from holding the magneto plate snug?
 
Yeah but I need it going for a fishing derby in a few weeks, so I might have to just pay the dealer top dollar, shipping wont be quick enough!

Another question while I await parts for the main issue...

If you look at number 35 in this schematic... The "packing washer"...Whats the best way to put these in? because I got two of them last week from the dealer (maybe gave me the wrong size) but when I put them in and screwed in the needle, it just shredded them apart.... I vaguely remember when I took the needle out the first time months ago, there was a little washer gasket on the engine side of the threads.. I don't see this in the schematic..

Here's the damage pictures...There's two cam's, one is from an old motor I had torn apart. The more damaged one is from this motor ( I did tap it with a screw driver to break it open a bit more though).

The flywheel, woodruff key and cam are all garbage now.

ps: dont mind the miss matched colours. Just bought this powerhead off ebay and wanted to see it run before I painted it.
