Hole in piston - 1987 454 Crusader

The engine was burning a lot of oil on the last trip out. I checked compression and found none in one so I tore it down. Turns out there is a hole in the piston.

What would cause this to happen?

While I have it a part should I proactively replace the head?

What else should I do while I have it torn a part to prevent it from happening again?

no, unless inspection indicates otherwise.

Ensure the fuel delivery and ignition systems are performing to spec.
