Hi,
I have a 29 year old Lowline Perkins 4-236. Its been running fine but had to rev it
very hard recently to get out of trouble.
I now have gray oil on the dipstick and water droplets under oil filler cap also
quite a bit of clear water in the cylinder head cover (2 or 3 teaspoons full).
Is this a head gasket problem? Does anyone know how I can find out before I
change it?

It sounds like you have a leak in the engine oil cooler.
Is there any bubbling in the expansion tank, steam in the exhaust or discolored coolant or low coolant? This would indicate a blown head gasket. Since the pressure in the cylinders is higher than the pressure on the coolant side you would expect to see signs of a head leak in the expansion tank and exhaust.
To avoid the engine seizing, up drain the engine oil ASAP and top up with fresh oil. Remove and pressure check the cooler. You will want to replace the cooler and run the engine with the fresh oil ASAP. Do not let it sit with the water in it or you will loose the engine to corrosion.
If the cooler checks out and there are signs as indicated above that the head gasket is blown replace the head gasket. Since the engine is old you will want to go ahead and check for cracks in the head and rebuild the head with new valves, guides etc;
My Perkins 4-236 has more than 20,000 hours and still runs great. The parts are available so it is worth the cost of rebuilding the head. You may want to have the injectors serviced at the same time since they will be removed during the rebuilding.

Hi Dwight

I have drained and replaced the engine oil and run the engine at 1000rpm for
about 3/4hr, the coolant looks clear and the oil on the dipstick stayed clear
except for the odd trace of what might be water.
As far as I am aware there is no engine oil. cooler, just a sea water fed gearbox
oil cooler, which is a separate from the engine oil, and another sea water cooler
for the
engine coolant.
I agree with you about servicing the head while I’ve got it off, I’m still hoping I
don’t have to tho!
Tomorrow I will run the engine up to working temperature, I didn't do this before
in case I caused more damage.
I’ll look in the header
tank to see what’s happening there.
