Spark plug wire pops off

This problem has been going on awhile. One of the spark plug wires pops off the spark plug.
The wire actually takes the screw-on terminal from the top of the spark plug, Threads not stripped. It was tight.
The spark plug is not set in a well. It is on the surface.

What is going on?
 
I have replaced the plug, but not the wire.
Could the threaded terminal be vibrating loose and spinning off the plug?
I tightened it on with pliers. The threads don't appear stripped.
To get the terminal out of the boot I have to thread a sparkplug into it in the boot. Pain in the ass.

Certainly is unusual. In all my years I've never seen one unscrew the plug terminal off the plug. Replace the boot and spring. You can't have two freaks in a ow.
