350 XL overheat and knocking...?

Just underway today. Port side ran up to 220 and alarmed.

I limped back at low rpm a short distance.

In the slip in idle I revved it up slightly and it alarmed again.
Pulled the hatch and heard a distinct knocking. Shut her down.

Couple observations:

Heat eX seemed very cool to touch. Odd?
Risers didn't seem elevated but the manifold temp I shot was well higher than the good engine.

185 vs. 212+

Had a new water pump kit installed over the winter on both engines. New belts

I was about to have the Risers and manifolds done ..maybe I got there too late?

Any insight on where to start and what might be the problem?

Thanks, ugh, I haven't had a chance to run the boat even this spring. 1997 with about 800 hours. Risers 4.5 years old .
 
I'd be inclined to start with the coolant level...

MakoMark thank you for your suggestion, engine was out of coolant due to a undone fill cap. Filled her up. Ran up to temp in the slip and stayed spot on .
Still have an intermittent knock or miss but substantially reduced. Im hoping
when I put a load on no issues arise.
