Help identifying this hydraulic steering cylinder?

The helm is a Hydraflex LD25 and the cylinder looks like a cross between an old Ultraflex and a Hynautic. It's a rectangular aluminum block with threaded end caps unlike the tubular ones I regularly see. The sticker says Performax, I think, but that gives me no results at all. I want to see about obtaining a seal kit before taking it into a hydraulics shop and asking for individual replacements.

Contacted Uflex and confirmed it is one of their old units, circa 1997. They were very gracious and I will be replacing only my cylinder as the helm and hoses don't appear to be leaking at this time.
