alternator pulley came off while underway

Had a weird thing happen this afternoon. Twin CH220 crusaders. Was heading home at about 2000rpm, had been underway for about an hour. All of a sudden the port side overheat alarm goes off and I noticed the temp was up to like 230. I cut the engine. Opened the hatch, found the alternator pulley on the floor, alongside the nut that holds it on, and the belt broken. My wife had to steer the boat up and down the river on one engine while I replaced the belt (thankfully had a spare) and put the pulley back on.

The port side engine is counter-rotating, and when I replaced this alternator six months ago I had an alternator shop take a standard alternator and put a bi-directional fan on it. Question is.. does a counter-rotating alternator also have opposite threads on the shaft, for the pulley to screw on in the opposite direction? Why would the nut just come off like that?
 
Just not installed tight enough. They all have RH threads.

I suggest you use some Lock Tite red thread locker when you put her on again.

Ayuh,..... You could use loc-tite,.... or stake it with a punch,.....

but I agree, it probably wasn't tight enough to begin with,....
