2001 mercury EFI 2.5L bad cylinders

I have a 2001 mercury 2.5L 200hp EFI that cylinder 2 busted the sleeve and pistons # 4 and # 6 are scored. Any ideas what could of caused this 1 3 5 all look good. Got block at machine shop getting fixed and am sending ECU to be tested and injectors to be cleaned
 
On the intake side towards the bottom of the sleave i would say if you were looking in the bore it would be about 8 oclock where the rod broke it.

I guess piston stuck in the bore and then the rod pulled out of piston and the rod hit the sleave breaking sleave and bending rod. But when I pulled the head off I just pulled piston out easy by hand so I'm not sure
