IACV came un-screwed in TBI

Was having idle issues, pulled off the flame arrestor and looked down and I could see the plunger end of the IACV all the way out. I pulled the whole IACV assembly out and the plunger end was all the way screwed out of the motor. What would cause this and is there a way to put it back together? I tried screwing it in by hand but it doesn’t seem to want to go. I put in a spare IACV and it working fine.

Is there any special instructions on installing the IACV (this is the 2[SUP]nd[/SUP] time I have had issues). I install first then plug in.
I was also told the automotive IACV is the same, I have been buying them at my local auto parts store.
