Anyone experience leaking O-rings? 20001 model year leaked coolant into exhaust and caused complete engine failureAnyone have similar leaking from O-rings in manifold or elsewhere?

12v2000 dd,

"See pictures!

12v2000 dd, 1480 hp - model year 2001. Less than 60 hours operation. Complete engine failure. Detroit / MTU does not acknowledge fault with O ring and coolant leak. Leak caused hydraulic of engine and ultimate failure.

Does anyone have experience with coolant leak from o-ring or similar situation?

Any help or comments appreciated.
