I have a Volvo Penta AQ151. I

I have a Volvo Penta AQ151. I recently replaced that
gaskets and impeller for the water system that cools
the heat exchanger (HE). This is always a challenge
getting everything adjusted so there are no leaks.
This is not my problem. I filled the HE with
coolant/water all the way to the top. When I bring the
engine up to temperature coolant leaks from the
radiator cap. Did I put too much coolant in the
system? Is the cap bad? Is something else wrong?

What is the general rule of thumb for coolant levels?

The HE should never be filled

The HE should never be filled with coolant mixture to the top. Room for expansion is necessary. How much? Typically the engine will find its own level or you can remove a half inch to start. Check or Change the cap is always a good practice if you want your cooling system well maintained. Cap pressure is important in maintaining proper operating temp of coolant. You can also mount an expansion tank similar to what cars have using the overflow tube on the neck of the HE.
