Honda 02 sensor questions

Okay guys had starboard 02 sensor go bad once again seems like they last about two years on my 2002 225hp ... Now new issue I found when changing this time the sensor tip was broken/gone any ideas what could cause this ? Also do I need to be concerned where pieces went in head ? Other then alarm going off no other problems when engine run at all... I am posting pic of my bad one and a pic of good oneo2 sensor.jpgPICT0573.jpg
 
WOW! Not ever seen that before. I have no idea what would have caused that except really high excessive heat or a manufacturing/assembly flaw. I would contact Honda tech support and see what they have to say. Logically, the missing piece should have rattled around in the water jacket and exited the exhaust tube.
 
yea I figured it would of rusted and rattled out of exhaust as well.. will have to see what this sensor does then I might make effort to contact honda
